what could account for her difficulty breathing and the symptoms closer to her pelvic region?

A woman slips in her kitchen while emptying her dishwasher and falls onto the knife she is putting away at the time. The knife enters her abdominal area just over her stomach. When the emergency team arrives they note that she is complaining of chest pain, stomach pain, has difficulty breathing, and is coughing.
