What can be done if you find false information in your medical records? The patient weighs 180 pounds and has recently lost 50 pounds. In the medical records it states she is morbidly obese weighing 280 pounds! Patient also has a hostory of diabetes and is currently on medications for diabetes. The first page states that she is diabetic, however the in the doctor’s notes regarding lab results and diagnosis the doctor states that the patient has no history of diabetes. What do you think and what should be done? To me this is not acceptable and very unprofessional. Thank you for your answers. This patient is an older female patient with a history of myelodysplastic syndrome and chronic abdominal pain with no known cause. I feel the doctors are not doing what they should to find a diagnosis for the pain and problems she is having.
Ask to update the chart and information.Ask the doctor to look over his/her remarks. You can have these notes sent to the patient for a small fee, then, go over them with the patient and the doctor.
